Where can I find a Claim form/Consent form/Medical information form?

You can get hold of a form by phone, post or by email through Customer Online. We’re currently working on getting them on to our website for you.

If you need a form of any kind, you can request one by: 

  • Calling us on your helpline number, which can be found in your handbook, on your membership card or by clicking here. Our opening hours are Monday to Friday 8am to 8pm and 9am to 5pm on Saturdays and bank holidays. Please note, we may record and/or monitor calls for quality assurance, training and as a record of our conversation.
  • Sending us a message through the Message Centre on your Customer Online profile. Click on the link to register if you haven’t done so already – it only takes a minute; you just need your date of birth and membership number. We aim to respond to all emails within 2 working days. If you’re on one of the few plans that don’t currently have access to Customer Online. (or if you aren’t able to register), you can email us using our Secure Message Centre.

How to return a form to us

You can return the form by:

  • Post, to the postal address above
  • Customer Online or our Secure Message Centre.
  • Fax, to 01892 508 456 (please note- this is our ‘standard’ fax number that goes to our central mail office. Please contact us by phone if your form needs urgent attention and you need a more direct fax number.)

When you’ve sent your form to us

If we’ve asked you for a form to establish if a condition is covered under the terms of your membership, we aim to assess it within two working days. We’ll then write to you to let you know whether or not the condition can be covered.
